At SMaRT Physical Therapy, we want to maximize your time with us. If you would like to save time on your first visit, please print the following patient documents below, fill them out, and bring them with you to your first appointment. Otherwise, allow extra time before your appointment to fill them out here.

Insurance Info

We gladly accept the insurance plans listed below. If your plan is not listed, please call us as we are always adding new insurance plans.

As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.

For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan. Please contact SMaRT Physical Therapy today for more information on our fees for service and payment details.

We have included most of the major plans with which we are in-network. While this is an extensive list, health plans do change regularly without prior notification. We recommend that you verify with your health plan what physical therapy benefits you have available. Please call the office to verify other insurance plans. SMaRT Physical Therapy offers a $85.00 per visit, self-pay fee if you choose to not use insurance.

FAQs

Can I continue sessions even after I feel better?

Yes, once you have completed your therapy sessions, your therapist will review your continuing home program.

We can make arrangements to meet you at your gym to work with you on determining what equipment may be beneficial, and what exercises would be helpful in your continued recovery.

How long will each of my sessions last?

Typically, the first session lasts for 1 hour 15 minutes. Each session after that may be 45-60 minutes. The amount of time will be based on your specific needs, and your therapist will discuss this with you.

I want to schedule an appointment – how do I do that?

Our scheduling is easy and we offer a convenient location and appointment times for you. We have morning and evening appointments available to meet the demands of your busy schedule. Call us today or request an appointment.

Will physical therapy hurt me?

Physical therapy is not meant to hurt, but to relieve pain. However, as you restore your mobility, pain levels, at times can fluctuate. You might experience some temporary soreness from using muscles that are weakened or recovering.

This is a normal response to therapy treatment. Good communication with your therapist will help minimize any discomfort. We want you on your way to quick pain relief and back to normal activities.
